Synovitis represents a critical turning point in haemophilia care, often developing silently, even in patients receiving prophylaxis. Without early identification and appropriate management, persistent synovial inflammation can lead to progressive joint damage, reduced mobility, and a long-term impact on quality of life.

In this medical educational programme, you'll explore the importance of early diagnosis and proactive management of synovitis in people with haemophilia A. Watch the video and view the accompanying slides to gain expert insights and practical guidance, highlighting how advances in treatment and monitoring can support improved long-term outcomes.

This programme will help you:

  • Recognise the clinical significance of subclinical bleeding and its role in synovitis development
  • Understand why synovitis represents the last reversible stage before irreversible arthropathy
  • Explore the role of routine joint assessment, including imaging modalities such as ultrasound
  • Learn how to optimise and individualise prophylaxis, alongside multidisciplinary interventions, to support joint health

Clinical takeaways

  • Bleeding, including subclinical bleeding, can occur despite patients receiving prophylaxis
  • Synovitis is the last reversible step prior to further deterioration leading to arthropathy
  • Routine assessment of joint health status is therefore essential and should include imaging, such as ultrasound, to facilitate the detection of early abnormalities
  • In people with haemophilia A and synovitis, prophylaxis should be intensified to resolve and/or reduce the synovial hypertrophy, alongside other measures (anti-inflammatory drugs, physiotherapy, etc.) to promote long-term joint health
  • Understand the pathophysiology of synovitis in haemophilia. How recurrent joint bleeds lead to synovial inflammation and progressive joint damage
  • Recognise synovitis as an early and partially reversible stage of haemophilic arthropathy and understand clinical relevance for long-term preservation and pain outcomes
  • Identify clinical signs, image findings and symptoms associated with synovitis, consider how they inform treatment optimization
  • Apply strategies to manage synovitis with the aim of reducing joint inflammation, preventing progression to irreversible arthropathy and improving quality of life

Dr Carmen Escuriola Ettingshausen is the Director of the Haemophilia Centre Rhein Main - HZRM, Frankfurt, Germany. She graduated in medicine at the Johann Wolfgang Goethe-University in Frankfurt from 1985 to 1992. After that, she became resident at the comprehensive care centre for Thrombosis and Haemostasis of the Children´s Hospital at University Hospital of Frankfurt in the department of Paediatrics until 1996, and obtained the Doctor title in 1995. She performed specialist´s training in Paediatrics at the University Hospital of Frankfurt from 1996 until 2000, and since then she was a staff member at the comprehensive care centre for Thrombosis and Haemostasis of the University Hospital of Frankfurt, department of Paediatrics until 2012. In 2012 she founded the Haemophilia Centre Rhein-Main. Dr Escuriola-Ettingshausen’s clinical interests include haemorrhagic disorders with a focus on haemophilia, thrombosis, congenital immunodeficiencies, and hereditary angioedema in both paediatric and adult patients. Her research interests are haemorrhagic disorders, particularly in treatment of people with haemophilia and those in treatment with inhibitors. Moreover, she is member of the German, Swiss and Austrian Society for Thrombosis and Haemostasis Research (GTH), International Society for Thrombosis and Haemostasis (ISTH) and the International Prophylaxis Study Group (IPSG).
